2025-2026 Missouri School Immunization Requirements

In accordance with Missouri state immunization law, all students are required to provide proof of immunization or a valid exemption prior to attending school. A written copy of the student's immunization record, provided by a healthcare provider, will be kept on file.

Submitting Immunization Records

Families may submit their student's immunization records in any of the following ways:

  •      • Drop off a printed copy at the school office with the building secretary.
  •      • Email a scanned copy of the record to the building secretary.
  •      • Fax the record to the school office (please include the student's full name and grade level).
